For all Damaged GM Body Claims, select and complete the form in its entirety, including the original invoice. Photos are required for all claims over $500 or 50% of cost.

 

Please note: All claims over $100 require approval before any repairs are made.​

For all Damaged Warranty Claims, select and complete the form in its entirety. Information required for processing includes:

• Original Mileage at Installation

• Date of Installation

• Mileage at Failure

• Date of Failure

• Original Repair and Replacement Repair Orders
